Incident Description

What happened? What did you see?
Code Abby was called at trailer ramp. Nurses and CCW
responded and client had an O2% of 80%. Client was given 1 IM
narcan and O2% rose to 95%. Client woke up and refused to come to
TED

What contributed to this incident?

Use of safe injection site

What were the immediate actions taken? (vital signs,
monitoring,
notifications, transfers to hospital)...
Vital signs were taken and O2% was at 80% and 1 IM
narcan given. O2% rose to 95% and client refused to come to TED
